Responsible for the organization and coordination of patient placement related activities in conjunction with Care Coordinators, Social Workers, and other health care members. Identifies systems or discharge problems in collaboration with interdisciplinary team. Performs care coordination administrative functions and participates in self-managed work teams. Requirements

  • Bachelor’s degree in health services administration or related field.
  • Excellent communication, collaboration, and organizational skills.
  • Knowledge of a variety of computer-based software programs such as Word, Excel, etc.

Nice To Haves

  • One to three years of experience in health care preferred.

Responsibilities

  • Collaborates with health care team to identify alternative placements for patients.
  • Interfaces with members of the health care team, patients, families and vendors, while making referrals and transfer arrangements.
  • Extracts supplemental information from the medical record in order to make appropriate referrals to facilities.
  • Documents progress of the discharge planning process in the medical record and in the MSM system.
  • Completes cases in the MSM after discharge.
  • Assists with completion of pertinent screenings and contacts appropriate regulatory agencies.
  • Establishes and maintains effective working relationships with facility representatives through telephone and/or direct contact. Maintains a supply of all necessary forms related to the discharge process on inpatient units.
  • Facilitates referrals by telephone and/or fax to rehabilitation, skilled-nursing, or home care facilities. Arranges for durable medical equipment (DME) as directed by care coordination team.
  • Identifies and recommends solution regarding systems discharge issues.
  • Complies with established departmental policies, procedures, and objectives.
  • Attends variety of meetings, conferences, seminars as required or directed.
  • Demonstrates use of Quality Improvement in daily operations.
  • Complies with all health and safety regulations and requirements.
  • Respects diverse views and approaches demonstrates Standards of Respect and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.
  • Maintains, regular, reliable, and predictable attendance.
  • Performs other similar and related duties as required or directed.
