About The Position

Central State Hospital (CSH) is a 277-bed Joint Commission accredited psychiatric inpatient facility located in Petersburg, Virginia. CSH has proudly been serving citizens of the Commonwealth since 1870. A new and modern CSH is currently under construction on the same property, with a scheduled completion date of 2027. CSH maintains the only maximum-security treatment facility in the state (111 beds) and has 166 beds available for individuals in the community who need inpatient care, as well as individuals in the criminal justice system. The Discharge Special Transport (Wage) position is responsible for supporting discharge planning and case management by assisting with escorting patients to community visits, clinical interviews, temporary visits, and day programs. This role fosters recovery-oriented care through respect, empathy, and encouragement of hope and positive expectations for persons served. The position collaborates closely with clinical social workers and community partners to facilitate smooth transitions into the community, provides timely feedback from escort activities, and supports the pre-discharge process by assisting in securing resources, services, and case management needs. The position requires accurate, person-centered documentation and reporting in compliance with hospital, Social Work Department, and Joint Commission standards, including maintaining logs of escort activities and submitting required reports within established timeframes. The incumbent participates in ongoing training and professional development, maintains a valid driver’s license and the ability to operate a state vehicle, and ensures all duties are performed in alignment with hospital policies and a commitment to continuous quality improvement and at least 90% compliance in all audits.

Requirements

  • Fundamental knowledge of the principles, practices, services and techniques of case management.
  • Demonstrated ability and skills to assist in coordinating resources for patients.
  • Ability to interview and gather pertinent information from patients and family.
  • Knowledge, skills and ability to communicate effectively orally and in writing.
  • Ability to perform duties at a fast pace with frequent change.
  • Ability to interact with other professionals.
  • Valid and current Virginia driver's license with the ability to operate a state vehicle.
  • Experience working with individuals with mental health conditions

Nice To Haves

  • Experience in social work, case management, and discharge planning is preferred.
