About The Position

The Discharge Planning Assistant provides support to the discharge coordination process. This role involves arranging ambulance or medical transportation, obtaining and sending patient information to external agencies, preparing various documents, and assisting with scheduling meetings and patient care conferences. The assistant also helps patients and families with specialty appointments, conducts follow-up calls, documents information in the EMR, and communicates any barriers in patient goal achievement to the care manager or clinical RN. Additionally, this role assists families with selecting home services, obtains authorizations and benefits for post-discharge services, and verifies insurance coverage.

Requirements

  • High school diploma or equivalent
  • 2+ years of work experience in a related job discipline OR equivalent combination of education and experience

Responsibilities

  • Arranges ambulance or medical transportation.
  • Provides support to the discharge coordination process.
  • Obtain, send and fax patient information to external agencies following hospital policy related confidentiality.
  • Prepares memos, letters and other documents including letters of medical necessity.
  • Assists with scheduling of meetings including patient care conferences.
  • Assist patient/family with specialty appointments as directed by care manager or clinical RN.
  • Completes follow up calls to clients and their care providers at the request of the care manager or clinical RN.
  • Able to elicit information during follow up calls and understands when to have care manager intervene.
  • Documents accurately and promptly in the EMR.
  • Communicates barriers in progress towards achieving patient goals with the care manager or clinical RN.
  • Assists family with selection process for home services covered by family payer(s) as delegated by care manager or clinical RN.
  • Obtains authorizations and benefits for post discharge services and documents in EMR.
  • Follows through with verification for insurance coverage, authorized days and benefits as directed by the care manager or clinical RN.
  • Participate with team members in quality improvement activities.
  • Demonstrate customer focused interpersonal skills, utilizing problem solving and unique family needs and dynamics.
  • Demonstrate family centered care in care team communications, rounds and meetings.
  • Demonstrate professional communication and conflict management skills.

  • Assists in maintaining supportive data bases at the direction of the manager of hospital care managers.
