About The Position

This position will be the main supporting role for the Case Management Service. This position supports the Case Managers, Medical Social Workers, Nursing staff, and other members of the health care team in meeting discharge planning and continuity of care needs for patients.

Requirements

• Associates degree in Business or Health related field and 3 years of relevant experience (preferably in a health related setting) or equivalent combination of education and experience
• Excellent written/oral communication, organizational and interpersonal skills required
• Working knowledge of computers
• PC and appropriate software knowledge
• Effective communication skills both oral and written
• Good interpersonal, motivational and strong organizational skills including time management required
• Customer service orientation
