Discharge Planner - SMRMC Social Work Services
About The Position
Saint Mary’s Health Network is seeking a Discharge Planner for Social Work Services at Saint Mary’s Regional Medical Center. Under the direction of an assigned Case Manager or Social Worker, this role coordinates activities of the Case Management Department and assists with the development and implementation of discharge plans. The position also assists with case management referrals and authorizations, maintains and coordinates Medicaid state submission forms for review and filing, and helps develop safe, patient-focused discharge plans by interviewing patients and families and gathering relevant information.
Requirements
- Knowledge of discharge planning, utilization management, and case management terminology and functions in managed-care and non-managed-care environments.
- Basic to intermediate computer skills and knowledge.
- High school diploma.
Nice To Haves
- Current BCLS certificate upon hire and maintained thereafter; listed as preferred.
- Associate degree or LVN license.
- Experience in a hospital setting.
Responsibilities
- Coordinate various activities of the Case Management Department under the direction of an assigned Case Manager or Social Worker.

- Maintain and coordinate Medicaid state submission forms for presentation to the on-site reviewer and file them according to facility procedures.
