About The Position

The Discharge Planner coordinates activities of the Case Management Department under the direction of the assigned Case Manager or Social Worker and assists with developing and implementing discharge plans. The role assists with case management referrals and authorizations, and maintains and coordinates Medicaid state submission forms for presentation to an on-site reviewer and filing according to facility procedures. As assigned, the Discharge Planner interviews patients and families and gathers information to develop a safe, patient-focused discharge plan.

Requirements

• Knowledge of Discharge Planning, Utilization Management, and Case Management terminology and functions in managed care and non-managed care environments.
• Basic to intermediate computer skills and knowledge.
• High school diploma.

Nice To Haves

• Current BCLS certificate upon hire and maintained thereafter; preferred.
• Associate degree or LVN license preferred.
• Hospital-setting experience preferred.
