Under the leadership of the Manager, Discharge Planning, the Social Worker/Discharge Planner will work within a fast-paced environment as a collaborative team member of an interdisciplinary team. The primary goal of Discharge Planning services is to support and enable patients and families to make timely transitions through the hospital system. The Social Worker/Discharge Planner does all of this in an effort with the goal of providing an exceptional patient and family experience. The Social Worker/Discharge Planner understands the Flow and Capacity challenges of a hospital system and works to achieve that the patients are in the right place, at the right time, receiving the right care. This role in our float pool will provide coverage to both our critical care and medicine departments. 2-5 years of experience in a critical care setting is required, including specific skills in crisis intervention, end of life care, and experience supporting goals of care discussions. The successful candidate will demonstrate an ability to balance collaborative practice with self-direction. They will excel at managing multiple tasks and setting priorities in a fast-paced environment.
