Seeking a Discharge Planner to join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Mitchell clinic.

Requirements

  • High school diploma or GED
  • A combination of behavioral health education and work experience totaling a minimum of four (4) years; OR a bachelor's degree in any field, plus one year of work experience in a behavioral health service delivery or a bachelor's degree in a behavioral health field
  • Valid Arizona driver’s license
  • Must be 21 years of age with minimum 4 years driving experience
  • Meet requirements of Terros Health’s driving policy
  • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
  • Must pass a TB Test.

Nice To Haves

  • Bilingual in Spanish is a plus (Additional Language Differential Pay Available)

Responsibilities

  • Serve as a liaison between the inpatient setting and the assigned clinician at the Outpatient site.
  • Work with Level I Inpatient Providers to review presenting clinical information.
  • Identify resources for successful discharge to the community.
  • Monitor TXIX adult GMH/SA Behavioral Health Recipients (BHRs) follow up with the outpatient provider.
  • Perform Discharge Planner activities in both inpatient and outpatient settings.
  • Connect individuals receiving Level I inpatient services with ongoing TERROS Outpatient treatment upon discharge.
  • Work with BHRs while in the hospital and during transitions.
  • Collaborate with hospital unit staff, the Outpatient team, family, RBHA care management, PCPs and Health Plans as clinically indicated to ensure effective transition to Outpatient or community services.
  • Reduce the risk of readmission to hospital.
  • Reduce the utilization of Crisis services.
  • Improve the coordination with the PCP and Terros psychiatric provider.
