Seeking a Discharge Planner to join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Mitchell clinic.

Requirements

High school diploma or GED
A combination of behavioral health education and work experience totaling a minimum of four (4) years; OR a bachelor's degree in any field, plus one year of work experience in a behavioral health service delivery or a bachelor's degree in a behavioral health field
Valid Arizona driver’s license
Must be 21 years of age with minimum 4 years driving experience
Meet requirements of Terros Health’s driving policy
Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
Must pass a TB Test.

Bilingual in Spanish is a plus (Additional Language Differential Pay Available)

Responsibilities

Serve as a liaison between the inpatient setting and the assigned clinician at the Outpatient site.
Work with Level I Inpatient Providers to review presenting clinical information.
Identify resources for successful discharge to the community.
Monitor TXIX adult GMH/SA Behavioral Health Recipients (BHRs) follow up with the outpatient provider.
Perform Discharge Planner activities in both inpatient and outpatient settings.
Connect individuals receiving Level I inpatient services with ongoing TERROS Outpatient treatment upon discharge.
Work with BHRs while in the hospital and during transitions.
Collaborate with hospital unit staff, the Outpatient team, family, RBHA care management, PCPs and Health Plans as clinically indicated to ensure effective transition to Outpatient or community services.
Reduce the risk of readmission to hospital.
Reduce the utilization of Crisis services.
Improve the coordination with the PCP and Terros psychiatric provider.
