About The Position

Under general supervision, coordinates the integration of Discharge Planning & Swing Bed level care. This position functions into the patient care, discharge and home planning processes with other hospital departments, external service organizations, agencies, and health care facilities; provides direct care to selected patients as appropriate; ensures compliance with quality patient care and regulatory standards.

Requirements

• Must possess assessment skills.
• Must exhibit sound judgment.
• Must be able to communicate effectively.
• Must be knowledgeable of community resources and be able to network effectively within the community.
• Must be knowledgeable regarding Medicare and AHCCCS regulations/requirements.

Nice To Haves

• Social worker or RN preferred, willing to train
• At least one (1) year experience working with medical patients preferred
