About The Position

Perimeter Behavioral of Springfield is looking for a compassionate, organized, and mission-driven Discharge Planner to help children, teenagers, and families move confidently through treatment and into successful aftercare. Requirements

Bachelor’s degree in social work, psychology, or a related field.
One to Two years of experience in an inpatient/outpatient mental health setting, or comparable experience; strong knowledge of clinical treatment principles and social casework preferred.
Successful completion of CPR and Crisis Prevention Intervention Training within 30 days of employment.

Responsibilities

Participate as an active member of the treatment team, including staff meetings, treatment planning meetings, and assigned committees.
Coordinate with therapists, utilization review staff, doctors, and insurance companies to support documentation, discharge planning, and continuity of care.
Help identify patient needs and work with the treatment team to ensure those needs are addressed.
Partner with families and treatment teams to encourage involvement in each patient’s care.
Communicate with referral sources, mental health professionals, and community agencies to support aftercare planning and successful transitions.
