About The Position

Valley Hospital Phoenix is seeking a dynamic and talented Discharge Planner to join their team of compassionate, dedicated professionals in the Clinical Services department. This role involves working closely with Inpatient Therapists to coordinate care and facilitate patient discharge planning. The position offers a per diem schedule with varying hours and the opportunity to work both independently and as part of a team in a fast-paced environment.

Requirements

  • Bachelor’s degree from an accredited college or university in social work, mental health, nursing, or related degree.
  • A minimum of three (3) years’ experience in a healthcare setting or managed care company, with experience in patient assessment, treatment planning, utilization management, and/or case management.
  • Knowledge of outpatient behavioral health treatment interventions, substance abuse rehabilitation, referral process for SMI services-Seriously Mentally Ill and the process of referral to ALTCS-Arizona Long Term Care, Guardianship and another other form of advocacy needed to provide excellent patient care.

Nice To Haves

  • L.M.S.W., LAC, LIASC, LCSW, LMFT, LPC, or other healthcare – related licensure
  • Master’s degree from an accredited college or university in social work, mental health, nursing, or related degree

Responsibilities

  • Assist the clinical team with coordination of care and discharge planning.
  • Have a thorough understanding of the patient’s treatment through communication with the treatment team.
  • Advocate for the patient’s access to services during treatment team meetings, communication with outpatient teams and referrals.
  • Meet with patients daily, complete referrals, SMI evaluations, staffing’s, and coordinate with outpatient providers.
  • Assist the team to ensure that the patient has a safe discharge placement, medication, and outpatient providers.
  • Verify safety within the home or place of discharge.
  • Provide patient education by facilitating psycho-education groups.
