About The Position

As part of our success and innovation in the field we are seeking qualified applicants for a PRN Discharge Planner position.

The Discharge Planner/Coordinator (BSW) provides discharge planning support to the psychiatric case-management department and supports psycho-educational group programming. Key responsibilities include, but are not limited to:
Initiates discharge planning and arranges aftercare appointments/referrals.
Completes all needed discharge paperwork with patients and supports at time of discharge.
May conduct psycho-educational groups and other groups as assigned.

Requirements

• Bachelor’s Degree in Social Work licensed by the State of Michigan required
• Strong verbal and written communication skills

Nice To Haves

• Previous administrative experience preferred - relevant internship experience will be considered as equivalent
