About The Position

Perimeter Behavioral Hospital of Arlington is seeking a compassionate and motivated Discharge Planner to join our team. This is an excellent opportunity to play a vital role in supporting patients' successful transitions from inpatient treatment to ongoing care and recovery.
Requirements
- Bachelor's degree in social work, Psychology, Counseling, or a related behavioral health field preferred.
- Minimum of one (1) year of experience in an inpatient behavioral health setting or a comparable healthcare environment strongly preferred.
- Knowledge of clinical treatment principles, case management, and discharge planning processes is highly desirable.
- Current Basic Life Support (BLS) certification.
- De-escalation Training: Handle with Care (HWC) certification.
- Ability to work flexible schedules and overtime as needed.
Responsibilities
- Participate as an active member of the multidisciplinary treatment team, including treatment planning meetings, staff meetings, and assigned committees.
- Collaborate with therapists, providers, utilization review staff, and insurance companies to coordinate discharge planning activities and required documentation.
- Assist with medical record completion and chart auditing activities.
- Assess patient needs and work with the treatment team to ensure appropriate services and resources are in place.
- Promote family involvement in the treatment and discharge planning process.
- Coordinate communication with referral sources, mental health professionals, and community agencies to develop comprehensive aftercare plans.
- Arrange discharge services and resources to support patient success following hospitalization.
- Maintain accurate and timely documentation in accordance with facility standards and regulatory requirements.
