Discharge Planner, Full time Days, Case Management
About The Position
This position involves discharge planning interventions with patients/families under the direction of Case Managers and Social Workers. Under the direction of the clinical staff, the Discharge Planner performs multiple activities that support the movement and transitions of patients out of the inpatient hospital setting, using current knowledge of hospital processes and community services. It involves frequent contact with patients and families, as well as multiple hospital staff and community resource, agency, facility and/or payor representatives. Acts as an internal resource to PFS around discharge planning, serves as a liaison to external case managers, review organizations, insurance companies, around discharge planning. This position requires flexibility of days and hours.
Requirements
- Bachelor's Degree in Social Work or a related social field.
- One year of experience in a healthcare setting for BSW graduates, or field placement in a healthcare setting preferred for new graduates.
- More than three years of recent experience in case management and/or utilization management for those with a Bachelor's in a social-related field.
- More than three years of recent experience in case management and/or utilization management for LVNs.
- Exemplary communication, coordination, and problem-solving skills, with an emphasis on customer service.
- Strong computer skills.
- Ability to manage activities and multiple priorities within complex systems.
- Tolerance for ambiguity, uncertainty, and change.
- Ability to function in an unstructured environment.
- Type minimum of 40wpm.
Nice To Haves
- Field placement in a healthcare setting preferred for new BSW graduates.
- Recent experience of more than three years in case management and/or utilization management preferred for Bachelor's in a social-related field.
- Recent experience of more than three years in case management and/or utilization management preferred for LVNs.
