About The Position
This position provides discharge planning as a member of an interdisciplinary team. Responsibilities include providing discharge planning, referrals, coordination with community providers, and other treatment-related duties. The role is responsible for patient progression of care, interviewing patients for initial discharge plans, and coordinating care to the next appropriate site of care. Requirements
- Bachelor's degree in related field required, or associate degree in related field with QHP required
- Strong writing and communication skills are required
