Disability Support Coordinator
About The Position
The Department of Human Services, Aging and Disability Services Division (ADSD) is seeking a Disability Support Coordinator (Developmental Disability Specialist II) to join our team. The Support Coordinator is responsible for providing direct service coordination for individuals with developmental disabilities that are receiving home and community-based services under the DD Medicaid Waiver. The incumbent in this position will ensure services are available and accessible for individuals to live and thrive in the Arlington community. As a Support Coordinator, the incumbent will be responsible for development of person-centered Individual Support Plans (ISPs) that direct outcomes across major life areas such as meaningful day, health and safety, employment, civic engagement, and community integration. The incumbent will join a collaborative team of Support Coordinators, program managers, and contract managers that facilitate person-centered planning for individuals supported by the program. The Arlington County Department of Human Services is committed to delivering client services in an effective, equitable, respectful, and trauma-informed manner. Benefits
- $2,500.00 gross signing bonus at hire
- $2,500.00 gross bonus at 12 months or upon completion of the probationary period
