About The Position

The Northern York South Simcoe Ontario Health Team (NYSS OHT) was one of the first OHT’s approved in Ontario. Our OHT is a partnership of several healthcare organizations who share a rich history of collaboration and an exciting vision for the future of healthcare in our communities. Members of our OHT include primary care, community service providers, homecare, community paramedicine, long-term care, hospital, and the mental health and addictions sector. Building on existing successful partnerships, the team has worked closely to plan and bring connected care to the communities we serve as an OHT. This position is permanent position. Note that the role's deliverables and responsibilities may evolve to match provincial and or local OHT priorities. Reporting to the Vice President, Academics and Community Health, and receiving strategic guidance from the Co-Chairs of the Joint Executive Steering Committee (JESC), the Ontario Health Team (OHT) Director serves as a key system leader responsible for building and maintaining strong relationships with internal and external partners to advance the health and wellness of the communities we collectively serve. The positions core focus will be leading the NYSS OHT. The Director will work in close collaboration with Southlake’s leadership team and broader community partners to support integrated planning, shared priorities, and coordinated system initiatives. The Director will foster a culture of collaboration across partner organizations and leverage strong project management and strategic planning expertise to support the development and execution of operational priorities, strategic initiatives, and key business processes. Working closely with the Joint Executive Steering Committee and established governance structures, the OHT Director will serve as a key driver of the OHT’s vision, mandate, and values, while acting as an important ambassador for the OHT across healthcare, community, and system partner organizations.

Requirements

  • Master’s degree in business administration, or healthcare administration.
  • 10+ years’ experience with five (5) years management experience, preferably within the health care sector.
  • Candidates with an equivalent combination of education and experience will be considered
  • Excellent communicator, strategic thinker, and team player with a passion for health system improvement and the ability to influence change
  • Experience applying a health equity lens to projects/initiatives aligned with the quadruple aim (population health, patient experience, per capita cost, provider experience) preferred
  • A successful record of accomplishment in change management and conflict resolution
  • Demonstrated participatory leadership, mentoring, and motivational skills with ability to guide/coach/influence multiple stakeholders
  • Excellent interpersonal and verbal/written communication skills
  • Knowledge and understanding of business practices, budgeting, procurement, human resource management, health and safety best practices and reporting requirements
  • Knowledge and understanding of community resources and partnerships, program planning, knowledge mobilization and communication/dissemination techniques
  • Superior report writing and presentation skills necessary for communicating complex information to a diverse array of groups
  • Proficient computer skills and the ability to work within a computerized environment
  • Strong time management, problem solving, decision making and organizational skills
  • Ability to exercise a high level of independence, integrity, professionalism, and confidentiality
  • Ability to manage competing demands and meet demanding timelines
  • Regular and reliable attendance is an expected requirement of this position.
  • Must be able to perform all necessary bona fide duties for the position, as outlined in the position guide.
  • Must review and be capable of performing duties as outlined in the Job Demands Analysis for the position as a condition of offer.
  • Proven ability to attend work regularly as per the Attendance Support Policy.

Responsibilities

  • Provide leadership, vision, guidance of the NYSS OHT, in alignment with the goals and objectives of the OHT, and overall direction of Ontario Health Works toward achieving consensus of health care providers for the NYSS OHT
  • Function as the key integrator of all OHT activities by providing leadership, oversight, and direction to all committee structures, enabling groups and councils to achieve key milestones and deliverables with the goal of realizing integrated service delivery
  • Improve new clinical population pathways and work to consistently measure and improve existing clinical pathways with a population health approach
  • Support clinical change management and communication initiatives to inform OHT stakeholders and the broader community as clinical population pathways are implemented
  • Lead and guide evaluation processes, prepare performance reports and other accountabilities required by Ontario Health
  • Provide oversight and direction for prudent financial and resource management for the OHT
