About The Position

The Director of Product Management for Care Coordination is Veradigm’s accountable product owner for care coordination and chronic disease management products and capabilities across Provider and Payer business segments. Reporting to the Vice President of Product Management, this leader defines and executes product strategy, from near-term release planning to a multi-year vision, grounded in customer insight, healthcare market dynamics, and company objectives. The portfolio is largely greenfield. The role builds the care coordination product from the ground up, determines priorities, and leads high-value opportunities from discovery through delivery, with an emphasis on scalable and repeatable products. The role begins as an individual contributor, owning the product directly until it is established, and is expected to lead a team as the portfolio matures. It drives alignment across Engineering, Design, Sales, Marketing, and Client Services, and is accountable for outcomes across organizational and product boundaries. Veradigm uses an empowered product team model rooted in SVPG principles: product managers own problems rather than features, cross-functional teams are empowered to find solutions, and success is measured by business and customer outcomes. The Director leads product strategy and discovery, product development and delivery, go-to-market enablement, team development, and regulatory awareness for care coordination products.
