About The Position

Greenbrook Medical is seeking a visionary Director of Value-Based Care Pharmacy to build its pharmacy program from the ground up. This role involves architecting the entire clinical pharmacy infrastructure for a rapidly growing senior-focused primary care organization. Responsibilities include formulary strategy, Part D cost optimization, HEDIS quality improvement, and provider education. The Director will collaborate with the Chief Medical Officer and physician teams to design the strategic roadmap for medication management, establish collaborative practice agreements, and create systems to drive cost savings and improve patient outcomes. The role requires direct patient care for complex chronic diseases, development of analytics, workflows, and team structure for scalability. The position is approximately 30% clinical, 40% program building, and 30% strategic/analytics. The role is based in the Tampa Bay area and requires regular in-person presence at clinic locations.

Requirements

  • Doctor of Pharmacy (PharmD) degree
  • Active Florida pharmacist license in good standing
  • PGY1 Pharmacy Residency
  • 4-7 years of clinical pharmacy experience
  • 3-5 years in leadership or program management roles
  • Demonstrated expertise in ambulatory care and chronic disease management (diabetes, hypertension, anticoagulation, heart failure)
  • Experience with managed care organizations, Part D cost reduction, and HEDIS quality measures required
  • Track record of building pharmacy programs from scratch or launching new clinical services
  • Strategic thinking balanced with hands-on execution
  • Financial acumen with understanding of medication economics, formulary management, and ROI tracking
  • Strong data analytics and reporting skills (Excel, Tableau, or similar tools)
  • Exceptional communication and presentation skills — able to influence physicians, train staff, and present to executives
  • Entrepreneurial mindset with comfort in ambiguity and rapid growth environments

Responsibilities

  • Build and lead pharmacy program infrastructure from the ground up, including formulary development, medication management protocols, and clinical workflows
  • Drive Part D and Part B cost optimization through therapeutic alternatives, formulary compliance, and patient assistance programs
  • Own pharmacy-related HEDIS quality measures and medication adherence improvement initiatives
  • Represent pharmacy at insurance partner meetings and contribute to managed care strategy
  • Provide direct patient care for complex chronic disease management under collaborative practice agreements
  • Conduct comprehensive medication reviews evaluating safety, efficacy, cost, side effects, and barriers to adherence, particularly for high-risk and polypharmacy patients
  • Reduce polypharmacy and improve medication adherence through targeted interventions aligned with value-based care goals
  • Manage chronic diseases (e.g., diabetes, hypertension, anticoagulation, heart failure) through proactive medication optimization and monitoring
  • Order, interpret, and apply laboratory results to guide medication therapy decisions and ensure safety and effectiveness
  • Improve transitions of care by identifying and resolving medication discrepancies post-discharge and reducing avoidable readmissions
  • Partner with care teams to ensure seamless coordination between primary care, specialists, and pharmacy services
  • Develop and deliver provider education programs on medication optimization and value-based care strategies
  • Build analytics dashboards and reporting systems to track program impact, cost savings, and ROI
  • Use data to identify opportunities to improve adherence, reduce cost, and elevate clinical outcomes
  • Visit clinic locations regularly for in-person provider engagement and patient care
  • Develop and mentor future clinical pharmacy team members, with the opportunity to build and lead a team as the organization scales
