About The Position

Optalis Healthcare & Rehabilitation at Leonard is seeking a compassionate, experienced, and organized Director of Social Work to lead our Social Services Department. This position is responsible for overseeing psychosocial services, supporting residents and families, ensuring regulatory compliance, and collaborating with the interdisciplinary team to promote quality resident outcomes.

Requirements

• Bachelor's Degree in Social Work required
• Current Michigan Social Work license preferred.
• Minimum of two years of experience in a skilled nursing, long-term care, or healthcare setting.
• Previous leadership or supervisory experience preferred.
• Strong communication, organizational, and problem-solving skills.
• Knowledge of Medicare, Medicaid, discharge planning, and long-term care regulations.

Nice To Haves

• Master's Degree preferred.
• Current Michigan Social Work license preferred.
• Previous leadership or supervisory experience preferred.

Responsibilities

• Direct and manage the Social Services Department.
• Complete psychosocial assessments and develop individualized care plans.
• Coordinate discharge planning and transitions of care.
• Provide counseling, support, and advocacy for residents and their families.
• Participate in care conferences and interdisciplinary team meetings.
• Ensure compliance with federal, state, and facility regulations.
• Assist residents in accessing community resources and support services.
• Maintain accurate and timely documentation.
• Supervise and mentor social services staff, as applicable.
