About The Position
Timberland Ridge, a skilled nursing and rehabilitation center in Fairlawn, OH, is currently seeking a Director of Social Services to join our dynamic management team. The Social Worker is responsible for managing and/or assisting with various resident services including the resident waiver program, PASSAR requirements, Level of Care (LOC) and Medicaid eligibility requirements as well as discharge coordination, care conferences and assisting with quality assurance for resident services.
Requirements
- Previous experience in a long-term care environment
- Compassionate and empathetic demeanor
Nice To Haves
- License to practice Social Work (LSW) in the State of Ohio
