Requirements

Bachelor's degree in Social Work Human Services, Sociology, Education, Psychology, Rehabilitation/Counseling or related field required
Minimum one year of experience working in skilled nursing required
Management Experience required
SSD Certification required

Nice To Haves

Master's degree a plus

Responsibilities

Provide for the physical, psychological, and social needs of the residents of the facility, motivating residents to make healthy adjustments to the skilled nursing center.
Assume administrative authority, responsibility and accountability to provide medically-related social services.
Implement social service interventions that achieve treatment goals, address resident needs, link social supports, physical care and physical environment to enhance our resident’s quality of life.
Coordinate admissions and discharge procedures.
Attend Interdisciplinary Team and Unit Care conferences.
Assess residents for psychosocial groups, and supervise implementation and documentation.
Interview new residents and assess the resident's psycho-social condition.
Participate in Interdisciplinary Team Meetings and Care Conference providing information on individual resident's psycho-social condition and prospects for discharge or movement within the facility.
Chart relevant social service information into the resident's chart.
Oversee and co-manage with the Director of Nursing the psychoactive committee review.
Discuss problems encountered by residents and assist with the implementation of solutions.
Conduct follow-up procedures to ensure resident satisfaction.
Note roommate problems and work toward resolution.
Make recommendations as to whether a resident can function effectively at home or at other levels within the facility.
Assist with discharge planning for homeward bound residents.
Attend in-service programs offered by the Nursing Home and may be requested to deliver in-services pertaining to Social Service functions.
Make all counseling decisions and referrals to outside agencies on a regular basis.
Maintain contact with staff from all Departments of the Nursing Home in attempting to remedy the diverse problems of Nursing Home residents.
Maintain contact on a continuing basis with the Director and Assistant Director of Nursing to exchange information on the status of individual residents.
