Requirements
- Master's degree (M.S.W.) in Social Work from an accredited University
- A minimum of five years related experience and/or training, or equivalent combination of education and experience.
- Experience working with the elderly in an adult living or long term care facility with knowledge of geriatric mental health services
- Knowledge of Medicaid/Medicare eligibility requirements, PDPM, MDS, Care Planning and basic medical terminology is required.
- Proficiency in computer skills, including MS Office, and Outlook
Nice To Haves
- Free supervision to become LCSW, or LPC to the qualified Candidate
Responsibilities
- Acts as facility liaison with county social workers, community services boards, Medicaid, SSI, Social Security, legal aide and other services
- Performs assessments of residents
