About The Position

Pilgrim Rehab and Skilled Nursing is seeking a Director of Social Services to join their collaborative and innovative team. This role involves assisting residents and their families with adjustments to illness, life changes, grief, loss, and placement through counseling and developing resident-centered care plans. The position also includes assisting with advanced directive planning, advocating for residents, attending facility meetings, completing documentation, collaborating on admissions, welcoming new residents, educating on financial needs and insurance, and orienting new employees regarding resident rights. The Director will also supervise the Social Services team.

Requirements

  • Must possess a college degree.
  • 2 Years’ experience in a long-term care facility.
  • LSW, BSW in the state of Massachusetts
  • Must be able to read, write, speak, and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
  • Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level at which they are currently functioning.
  • Must be able to relate information concerning a resident's condition.
  • Must not pose a direct threat to the health or safety of other individuals in the workplace.

Nice To Haves

  • Bachelor's in social work preferred.

Responsibilities

  • Demonstrates an awareness of and respect for diversity in interactions with residents, personnel, families, and stakeholders.
  • Assists short-term and long-term residents and their families in adjusting to illness, life changes, grief, loss, placement, and depression through counseling and developing resident-centered care plans.
  • Assists with advanced directive planning in collaboration with nursing and medical services.
  • Responds to resident and family needs by advocating on behalf of the resident's wishes.
  • Attends appropriate facility meetings (e.g., Care plan meetings, welcome meetings, Medicare meetings) on behalf of residents.
  • Protects residents from abuse, neglect, and misappropriation of property.
  • Completes documentation per federal and state guidelines and sends required documentation to appropriate agencies and physicians.
  • Assists in ensuring accuracy in census, collaborating with nurse liaison on prospective residents, touring prospective residents/families, and conducting PASAAR screens.
  • Meets and welcomes new residents to the facility.
  • Educates residents and families regarding financial needs, monitors and assists with insurance verification and transitions of payment options, issues Medicare A & B non-coverage cut letters, and collaborates with the business office for payment issues.
  • Assists with orientation of new employees as it relates to resident rights.
  • Supervises Social Services team.
