About The Position

The Director of Social Services is responsible for initiating treatment plans, completing psychosocial assessments, and developing discharge plans for patients. This role involves advocating for patient rights, making appropriate referrals to community resources, and collaborating with healthcare professionals in treatment team meetings. The Director also facilitates social services groups, follows up on mental hold processes, and assists patients with various applications. Additionally, this position may involve writing behavior modification plans and letters for guardianship referrals.

Requirements

  • Licensed with Master’s Social Work (LMSW) / Professional Counseling (LPC)
  • Experience supervising staff
  • Successful completion of background check

Responsibilities

  • Initiate treatment plan, complete social services portion of treatment plan, and complete a tentative discharge plan within 24 hours.
  • Review patient progress in treatment plan and complete psychosocial assessments on each patient within 72 hours.
  • Review patient progress weekly.
  • Conduct discharge meetings with patients and families to discuss discharge and crisis plans, as well as patient and family expectations.
  • Advocate for patient’s rights and ensure patient needs for services during admission and post-discharge are offered/referred.
  • Make appropriate referrals to community resources and schedule follow-up appointments.
  • Refer patients to in-pt. or out-pt. substance abuse treatment programs, partial hospitalization programs, counseling, PCP, med management, CBRS, case management, SUD treatment, PCS services, and home health, etc.
  • Review prospective patient charts and provide opinions to the doctor regarding the appropriateness of admission.
  • Participate in treatment team meetings with each psychiatrist bi-weekly.
  • Facilitate social services group on scheduled days.
  • Meet with and call the DE's to discuss patient progress and mental hold status.
  • Follow up on the mental hold process to ensure timely filing of paperwork and that DE's meet their deadlines.
  • Fill out and file physician's holds.
  • Write personalized behavior modification plans for patients as needed; write letters for patients; write letters for guardianship referrals.
  • Assist patients in filling out applications for Medicaid, Social Security, Indigent Funding, and Prescription assistance.
  • Fill out PASRR and send all chart information for approval for level 2 screening.
