About The Position

Recruiting a Social Services Director that will work to attain, maintain or enhance the psychological and social well-being of each resident and provide psychosocial support to residents and families.

Requirements

  • Masters degree in social work or related human service field with at least one year supervised experience in a healthcare setting.
  • OR Bachelor’s degree in social work or related human service field with at least two years supervised social work experience in a healthcare setting.
  • Ability to communicate in English effectively through verbal and written means.
  • Ability to work under stress and in emergency situations.
  • Understand principles of growth and development for the entire lifespan with the ability to provide care appropriate for adult and geriatric residents as appropriate to the resident population.

Responsibilities

  • Perform duties in accordance with applicable federal and state laws and professional standards.
  • Keep confidential all resident information.
  • Participate in the facility Performance Improvement Process.
  • Provide a psychosocial assessment for each resident within seven days of admission, annually and/or upon change condition.
  • Maintain quarterly progress notes regarding: reason for admission, goals for discharge, mental/mood status, other psychosocial concerns, as well as basic family concerns and issues.
  • Collaborate with multi-disciplinary team to develop a comprehensive care plan.
  • Review and/or update each resident’s care plan at least every 90 days or on a as needed basis, based on change in condition.
  • Provide direct psychosocial intervention by: implementation of care planned interventions, planning and conducting group and facility-wide psychosocial interventions.
  • Assist the resident’s family in coping with: problems associated with Skilled Nursing placement, the physical illness/disability of the resident, and the grieving process.
  • Educate the resident and family, friends and staff about resident’s rights.
  • Take a proactive role in the facilitation of the resident/family council.
  • Assist with the resident’s communication with family, the staff and outside agencies.
  • Encourage the participation of the resident and other significant decision makers in the resident’s care planning process.
  • In conjunction with the rehabilitation department, make arrangements for needed adaptive equipment devices or community referrals.
  • Educate residents and their families about Durable Power of Attorney for healthcare or other forms of advance health directives and provides assistance in documentation of advance directives.
  • In-service staff regarding psychosocial issues, the role of the social worker in Skilled Nursing and resident’s rights.
  • Provide the resident and families with community resource information regarding discharge options.
  • Work with the resident, the family and the other disciplines to plan for a safe discharge to home or another level of care.
  • Document discharge plans.
  • Promote adherence to our Code of Business Conduct by monitoring employee performance, as well as, identify and respond to compliance issues.
  • Perform all other duties as assigned.
