Requirements

Active Social Worker License in the State of New Jersey (required)
Strong leadership and interpersonal skills
Passion for improving the lives of seniors in a healthcare setting

Responsibilities

Lead and manage social services for residents in a skilled nursing setting
Provide guidance, advocacy, and emotional support to residents and families
Collaborate with interdisciplinary teams to enhance resident care and quality of life
Extend support services to our assisted living residents as needed
