This position supports residents through transitions, assists with problem resolution, coordinates resources, and partners with multiple departments to ensure residents receive the support they need to thrive.

Requirements

  • Experience working with older adults or in a senior living environment required
  • Bachelor's or Master's degree in Social Work preferred, but not required
  • Strong interpersonal, communication, and conflict-resolution skills
  • Ability to maintain confidentiality and handle sensitive situations with professionalism and compassion
  • Strong organizational and documentation skills
  • Proficiency with electronic record systems and Microsoft Office applications
  • Ability to work collaboratively across departments

Responsibilities

  • Develop strong relationships with residents and families while serving as an advocate for resident needs
  • Serve as a primary point of contact for resident concerns and facilitate resolutions
  • Assist residents and families through life transitions, including changes in health status, level of care, rehabilitation needs, and move-outs
  • Participate in resident assessments and care planning discussions
  • Support new residents during move-in and adjustment to community life
  • Collaborate with resident committees and serve as a liaison to the Resident Association
  • Connect residents and families with appropriate community resources and support services
  • Maintain documentation of resident interactions and follow-up activities
  • Participate in resident orientation programs and Resident Care Committee meetings
  • Support families during end-of-life transitions and coordinate memorial service communications
  • Collaborate with internal departments and external providers to support residents' social, emotional, spiritual, and healthcare needs
  • Assist with volunteer programs, resident communications, and special projects as needed
