About The Position

Leads the day-to-day operations of high quality, person-centered care within the community, completing residential placement and ongoing nursing processes; collaborating with physicians and multidisciplinary team members; providing social, emotional, physical, and emotional support to residents, friends, and families; assist with hiring, training, and supervising assigned care team members. The Nurse Manager communicates daily with the Executive Director, assumes authority to carry out the licensee or Executive Director’s responsibilities in their absence.

Requirements

  • Clinical Skill, Customer Service Skills and Phone Etiquette, Infection Control, Nursing Skills, Physiological Knowledge, Administering Medication, Teamwork, Multi-tasking, Listening, Verbal Communication, Health and Wellness Promotion and Maintenance; Experience in Senior Living Management with Special Knowledge/Expertise in Dementia Care.
  • Compassion/skills to help people focus.
  • High School Diploma or equivalent required.
  • A minimum of 3 to 5 years previous experience as an LPN required.
  • Must have a desire to work with Dementia/Alzheimer’s.
  • Must be able to read, write, and speak English fluently.
  • Ability to work flexible hours as needed.

Responsibilities

  • Sets the tone and standards for both person and relationship-centered care reflecting the core values of Vibrant life.
  • Leads overall day-to-day medical operations of Vibrant life.
  • Steps in as a Med Tech/ Resident Assistant when needed to adhere to licensing requirements and Vibrant Life Standards
  • Identifies resident care requirements by establishing personal rapport with potential and actual persons in our care and other people in a position to understand care requirements.
  • Establishes a compassionate and vibrant environment, providing emotional, psychological, and spiritual support to the person, their friends, and families (mind, body, spirit)
  • Establishes proficiency in State regulations and other licensing requirements.
  • Directs and supervises scheduling of direct care employees within established guidelines.
  • Takes the lead in training care staff and partners with other management team members in ongoing staff recruitment, retention, and ongoing staff development efforts.
  • Promotes person-centered care goals; teaching, the person (as able), staff, family, and friends to understand condition, medications, and self-care skills; answering questions.
  • Assures quality of care by adhering to established dementia care standards; measuring health outcomes against person-centered care goals and standards; making or recommending necessary adjustments; best practice nursing standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.
  • Resolve the person’s problems and needs by utilizing multidisciplinary team strategies.
  • Maintains safe and clean working environment by complying with procedures, rules, and regulations; calling for assistance from other management team members and/or health care support personnel.
  • Protects people in our care and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations
  • Documents individual care service plans by charting in the person’s record and department records
  • Maintains continuity among staff teams by documenting and communicating actions, irregularities, and continuing needs.
  • Maintains individual and family member’s confidence and protects operations by keeping information confidential.
  • Maintains nursing supplies inventory by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies; verifying receipt of supplies; using equipment and supplies as needed to accomplish job results.
  • Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies
  • Maintains a cooperative relationship among health care teams (both internally and externally) by communicating information; responding to requests; building rapport; participating in team problem-solving methods.
  • Contributes to team effort by accomplishing related results as needed.
  • Works with Executive Director to assure efficient, cost-effective operation of the community.
  • Ensures operation of equipment by completing preventive maintenance requirements; following manufacturer's instructions; troubleshooting malfunctions; calling for repairs; maintaining equipment inventories; evaluating new equipment and techniques
  • Consistently meets/exceeds regulatory service/care requirements.
  • Provides consultation/phone triage after business hours to staff when nursing needs arise, beyond scope of staff knowledge and/or sets up rotation of lead staff to triage emergencies to appropriate medical and management personnel on call.
  • Attend and participate in meetings and training.
  • Complete and maintain all state required forms for the nursing department.
  • Carrying out referrals for DME services
  • Carrying out primary physician, Hospice, PT and PACE orders
  • Making medication changes as needed
  • Communicating with Pharmacies regarding medication orders
  • Narcotic control by tracking, destruction, and refills
  • Monitoring non-controlled medications such as discontinuation, charting into ECP
  • Order labs and report results
  • Assist with scheduling care staff, hiring care staff to stay in state compliance.
  • Other duties as assigned by the Executive Director
