About The Position
Functional Pathways is currently hiring at our newest location, Wesley Willows in Rockford, IL, for a full-time Director of Rehab. Wesley Willows is a senior living community in Rockford, IL, that has been serving seniors since 1966. Located on a beautiful campus, Wesley Willows offers a full continuum of care, including independent living, assisted living, memory care, skilled nursing, and rehabilitation. Requirements
- Must have an Associate (PTA or COTA) or a minimum of a Bachelor (PT, OT or SLP) of Science degree from an accredited Occupational Therapy or Physical Therapy, or Masters of Speech-Language Pathology.
- Must meet all certification and licensure requirements as established by state and national board associations.
- Active state license.
- Completion of all required screenings, certifications, and licensure.
- Excellent organizational and time management skills.
- Excellent written and verbal communication skills.
- Ability to collaborate with interdisciplinary teams.
- Willingness to engage in ongoing professional development and continuing education activities.
Nice To Haves
- Two years clinical experience preferred, with experience in geriatric care.
- Management experience in the skilled setting, preferred.
Responsibilities
- Supports the RM, RVP, and Operations Team to ensure the efficient operation of the facility rehab department.
- Serves as the liaison between Functional Pathways, the facility, and rehab staff.
- Coordinates and integrates care.
- Supervises rehab staff.
- Maintains accurate and timely documentation.
