About The Position

At Schofield, the Director of Rehab Operations is a key leadership role responsible for guiding rehabilitation services across Schofield Residence, Greenfield Health and Rehab, Schofield Home Care, and Schofield Adult Day. This position provides strategic direction and day-to-day operational leadership while ensuring high-quality care, regulatory compliance, strong team performance, and positive outcomes for the individuals we serve. The Director will collaborate closely with facility and corporate leadership to strengthen programs, develop staff, improve efficiency, and support the continued growth of rehabilitation services. We’re looking for a compassionate, experienced rehabilitation leader who is passionate about helping older adults maintain their independence and quality of life. The primary work location is Schofield Residence, with oversight across the organization’s additional rehabilitation programs.

  • Must be able to move intermittently throughout the work day and travel between organizational programs and locations as required.

  • Must meet the general health requirements set forth by the policies of the facility.
  • Must possess the ability to perform the essential functions of the position with or without reasonable accommodation.
  • Must possess a current license and be registered to practice in the State of New York as a Physical Therapist or Occupational Therapist.
  • Must be a graduate of an accredited Physical Therapy or Occupational Therapy program.
  • Minimum of five years of clinical rehabilitation experience required.
  • Minimum of three years of leadership or multi-site rehabilitation management experience required.
  • Demonstrated experience overseeing rehabilitation operations, staffing, quality outcomes, productivity, and financial performance.
  • Strong knowledge of rehabilitation regulations, documentation requirements, billing practices, and PDPM.
  • Strong interpersonal, communication, leadership, and organizational skills.
  • Ability to develop and maintain effective working relationships with interdisciplinary teams and organizational leadership.
  • Ability to analyze performance data and implement strategies to improve quality, productivity, efficiency, and financial outcomes.
  • Ability to effectively manage multiple programs, priorities, and teams across different locations.
  • Possess the ability to deal tactfully with various personalities and exhibit maturity, professionalism, patience, and sound judgment.
  • Must demonstrate compassion for older adults and a commitment to promoting quality of life and independence.

  • Experience in a skilled nursing, long-term care, home care, adult day, or other healthcare setting preferred.

Responsibilities

  • Provide strategic leadership, guidance, and support to rehabilitation departments across all four programs.
  • Oversee scheduling, staffing, supervision, and daily rehabilitation operations at Schofield Residence.
  • Develop and implement policies, procedures, best practices, and operational standards across programs.
  • Monitor quality outcomes, productivity, financial performance, staffing, and departmental metrics.
  • Provide regular performance reporting and dashboards to organizational leadership.
  • Lead quality assurance initiatives and ensure compliance with State, Federal, and payer requirements.
  • Ensure high-quality therapy services, documentation compliance, billing accuracy, and PDPM alignment.
  • Collaborate with leadership on strategic initiatives, staffing models, program growth, census growth, case management, and discharge planning.
  • Support recruitment, onboarding, retention, coaching, performance evaluations, and professional development of rehabilitation staff.
  • Oversee departmental budgets, productivity, resource allocation, and operational efficiency.
  • Lead staff education, interdisciplinary collaboration, and performance improvement initiatives.
  • Partner with the Assistant Director of Rehab and provide direct supervision to the Assistant Director and Schofield rehab team.
  • Advance operational alignment between Greenfield Health and Rehab and Schofield Residence.
  • Lead development of the PDPM Medicaid Long-Term Care program and expand rehabilitation specialty positions.
  • Coordinate a shared Lineage Care Therapist role across all sites.
  • Improve Home Care scheduling efficiency, including therapist assignments by geographic area.
  • Partner with Quality Assurance leadership to advance Home Care quality and functional outcomes.
  • Support and strengthen the Adult Day therapy program while providing leadership to its therapy staff.
  • Promote teamwork, integrity, clinical excellence, and adherence to the mission, values, and policies of Schofield and its affiliated organizations.
