About The Position

The Director of Population Health & Value-Based Care provides strategic and operational leadership for developing and implementing Rome Health’s emerging population health strategy. The role builds a practical, sustainable approach to improving clinical outcomes, patient access, care coordination, quality performance, and organizational readiness for value-based reimbursement. As the organization develops its population health capabilities, the Director assesses current capabilities, establishes priorities, coordinates existing resources, develops performance measurement systems, and creates a multi-year roadmap. This is a hands-on leadership role balancing strategy with implementation in a resource-conscious environment. The Director leads population health strategy and program development; supports progression toward appropriate value-based payment models; engages physicians and clinical teams in practical workflows and quality improvement; develops actionable reporting, dashboards, and performance measures; and strengthens care management, transitional care, chronic disease management, and coordination across care settings. The role also aligns initiatives with quality, patient safety, preventive care, access, and community health priorities, and develops relationships with community and healthcare partners as appropriate. Initial priorities include completing a population health readiness and capability assessment; establishing baseline quality, utilization, clinical, and financial measures; developing a prioritized three-year roadmap; identifying priority populations; establishing an initial physician/provider performance dashboard; evaluating care-management and transitional-care workflows and value-based payer arrangements; implementing two to three measurable improvement initiatives; and establishing regular reporting to senior leadership. The Director will recommend future staffing, technology, analytics, and program investments based on demonstrated needs and priorities.
