About The Position

The Director of Patient-Centered Medical Home Model Program is responsible for developing, strategizing, implementing, and maintaining program initiatives. This role involves assessments for evaluation, ensuring organizational standards of satisfaction, quality, and performance to meet PCMH practices. A key aspect is ensuring the security and privacy of data within associated systems and leading PCMH transformation and ongoing compliance with accreditation standards. The position also involves collecting and submitting data for American Indian Medical Home (AIMH) designation, preparing documentation for PCMH recognition/accreditation, and monitoring adherence to policies and care delivery standards. This role supports care coordination, promotes patient-centered care, tracks clinical quality metrics, leads quality improvement initiatives, identifies gaps in care, and facilitates interdisciplinary team meetings. Training staff on PCMH principles and serving as a liaison between leadership, providers, and community resources are also core functions. The Director will utilize the Electronic Health Record (EHR) system for data extraction and analysis, ensuring data integrity.

Requirements

  • Three (3) years of direct work experience in quality improvement in a health care setting, data analytics or care coordination in a Primary Care setting.
  • Bachelor’s degree in Healthcare Administration, Nursing, Public Health, or closely related field.
  • Must obtain PMCH certification (Joint Commission, AAAHC, or NCQA) within two (2) years from date of hire and maintain certification.
  • Valid, unrestricted insurable driver’s license.

Responsibilities

  • Support care coordination across providers, specialists, and community resources.
  • Promote patient-centered care, including shared decision-making and self-management support.
  • Track, analyze, and report clinical quality metrics.
  • Lead quality improvement initiatives.
  • Identify gaps in care and assist with corrective action plans.
  • Facilitate interdisciplinary team meetings with stakeholders.
  • Train staff on PCMH principles and workflows.
  • Serve as a liaison between leadership, providers, nurses, and community resources.
  • Utilize the Electronic Health Record (EHR) system to extract and analyze data.
