This position is responsible for developing, strategizing, implementing, and maintaining program initiatives related to the Patient-Centered Medical Home (PCMH) model. The role involves ensuring the quality, satisfaction, and performance of PCMH practices, as well as managing data security and privacy. The Director will lead PCMH transformation, ensure compliance with accreditation standards, and manage data submission for designations like the American Indian Medical Home (AIMH). Key responsibilities include preparing documentation for PCMH recognition, monitoring adherence to policies, supporting care coordination, promoting patient-centered care, tracking clinical quality metrics, leading quality improvement initiatives, identifying care gaps, facilitating team meetings, training staff on PCMH principles, and serving as a liaison between various stakeholders. The role also requires utilizing the Electronic Health Record (EHR) system for data analysis and ensuring data integrity. The position is located within the Navajo Nation, and FDIHB has a Navajo/Indian Preference in Employment Policy.
