About The Position

This position is responsible for developing, strategizing, implementing, and maintaining program initiatives, assessments for evaluation, and organizational standards of satisfaction, quality, and performance to meet PCMH practices. The role ensures the security and privacy of data in all FDIHB, Inc. associated systems and leads PCMH transformation and ongoing compliance with accreditation standards. The Director will collect and submit data to AHCCCS for American Indian Medical Home (AIMH) designation, prepare and manage documentation for PCMH recognition/accreditation, and monitor adherence to policies, workflows, and care delivery standards. This role supports care coordination across providers, specialists, and community resources, promotes patient-centered care including shared decision-making and self-management support, and tracks, analyzes, and reports clinical quality metrics. The Director will lead quality improvement initiatives, identify gaps in care, assist with corrective action plans, and facilitate interdisciplinary team meetings with stakeholders. Additionally, the role involves training staff on PCMH principles and workflows, serving as a liaison between leadership, providers, nurses, and community resources, and utilizing the Electronic Health Record (EHR) system for data extraction and analysis, ensuring accurate documentation and data integrity. Other duties as assigned.

Requirements

  • Three (3) years of direct work experience in quality improvement in a health care setting, data analytics or care coordination in a Primary Care setting.
  • Bachelor’s Degree in Healthcare Administration, Nursing, Public Health, or closely related field.
  • PMCH certification (Joint Commission, AAAHC, or NCQA).
  • Valid, unrestricted insurable driver’s license.
  • Resumes and references are required.
  • Must be an enrolled member of the Navajo Nation (for primary preference) or a member of other federally-recognized Indian tribes (for secondary preference).
