About The Position

The Primary Care team is seeking a Social Worker to join their thriving team! This is a fully remote position, with occasional onsite meetings in the greater Charleston area. Candidates within commuting distance preferred.

Requirements

• Master's degree in a social work or social welfare program accredited by the Council on Social Work Education required.
• Masters in Social Work (MSW) licensure by the South Carolina Board of Social Work Examiners required.

Nice To Haves

• Certification in Case Management preferred.

Responsibilities

• Assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes.
• Completes an assessment on the patient and caregivers throughout the hospitalization and outpatient follow-up as needed.
• Identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge.
• The Social Worker (MSW) bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions.
