About The Position

The Director of Patient Care is a registered professional nurse responsible for ensuring the quality of patient care in the home. This role involves the overall administration of clinical departments, monitoring staffing and productivity, and coordinating care with the interdisciplinary team, patient/family, and referring Agency. The position requires promoting the company's mission, vision, and values, and ensuring compliance with State and federal regulations. The Director will supervise and direct subordinates to ensure quality and continuity of services, execute staff education programs, and assist in hiring quality home health personnel. They will also participate in business plan formulation, budget planning, and oversee QAPI and utilization review activities. Compliance with third-party payors, infection control, and effective communication are also key aspects of this role. The Director will evaluate agency programs and personnel, assist in community needs assessment and program planning, market the organization, foster community relations, and act as a resource for the Hospice division leadership.

Requirements

Minimum 2-3 years’ experience as a registered nurse obtained within the last 36 months.
Evidence of progressive leadership responsibilities, including fiscal management, is required.
Excellent communication skills.
Excellent interpersonal skills necessary to communicate effectively with various personnel when identifying and resolving sensitive and complex patient or administrative issues.
Nursing skills are defined as generally accepted standards of practice.
Excellent interpersonal skills and ability to communicate effectively.
Demonstrates proven decision-making skills.
If required to make visits, Hepatitis profile.
Must read, write, and comprehend English.
License and current registration as a Registered Nurse.

Nice To Haves

One year of experience in home health care is preferred.
Management experience preferred.

Responsibilities

Promote /exemplify the Company's mission, vision, and values at all times.
Direct and coordinate clinical departments and branches; assume responsibility for continuity, quality, and safety of services delivered in compliance with State and federal regulations. (Conditions of Participation)
Participate in activities relevant to professional services, including development of qualifications and assignment of Agency personnel for assigned agencies.
Supervise and provide direction to subordinates, in an effort to ensure quality, compliance with Plan of Care and assessment and reassessment of patient's needs and continuity of services by appropriate health care personnel.
Ensure execution of all job functions and job descriptions for service staff (direct and supervisory).
Execute a consistent companywide orientation and staff education program.
Assist with the interview process to select and hire quality home health personnel.
Identify areas of training needed to develop any staff member further.
Participate with the Administrator and Chief Executive Officer in the formulation of a business plan, i.e., Strategic Goals, resource allocation plan(s), and a yearly budget.
Assure productivity requirements of full-time, specific package and contract staff.
Assist the Administrator with planning activities to develop administrative and clinical policies and practices for the agencies assigned.
Oversee all QAPI and utilization review activities, including timely initiation of corrective action plans and controls.
Promote compliance with all fiscal intermediary and/or other third party payors, through education, coaching, and other assistance as necessary.
Monitor systems and identify problem areas to the Administrator both verbally, electronically, and through written reports.
Meet mandatory continuing education requirements of the Agency/licensing board.
Promote and educate regarding the concepts of infection control and universal precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
Use effective interpersonal relations and communication skills; facilitate other team members' use of these skills to achieve desirable outcomes.
Evaluate the assigned agency's program, services, and field personnel.
Assist in evaluating community needs and planning programs and services to meet identified needs.
Market the Organization and foster community relations.
When appropriate, participate in state and national organizations, meetings, seminars, workshops, and activities related to health professions and health care services.
Proactively identify issues impacting patient care and/or office operations and involve operations management/clinical management as indicated.
Facilitate implementation of Company initiatives and policy, regulations, processes, and procedures changes.
Act as a resource to provide support and expertise to the Hospice division leadership.
