You’ll lead a team of skilled nurses, aides, and interdisciplinary professionals, ensuring that every patient receives exceptional, individualized care. Your leadership will directly shape the quality of life for patients and the support their families receive—turning our mission into a daily reality.

Requirements

  • Current RN license in good standing; BSN required, MSN preferred.
  • 5+ years of nursing experience, with at least 2 years in hospice or palliative care leadership.
  • Deep knowledge of hospice regulations, clinical best practices, and interdisciplinary care models.
  • Exceptional communication skills and the ability to lead with empathy and clarity.
  • Strong organizational skills with a track record of managing multiple priorities in a fast-paced environment.
  • A passion for end-of-life care and a commitment to honoring patients’ dignity and choices.

Responsibilities

  • Lead and inspire the hospice nursing team, fostering a culture of empathy, accountability, and clinical excellence.
  • Oversee patient care plans, ensuring they meet regulatory standards and reflect each patient’s unique needs and wishes.
  • Collaborate with physicians, social workers, chaplains, and volunteers to deliver holistic, patient-centered care.
  • Monitor quality metrics, conduct case reviews, and implement process improvements to enhance care outcomes.
  • Manage staffing schedules, training, and performance evaluations to maintain a highly-functioning, compassionate team.
  • Ensure compliance with state, federal, and accreditation requirements, including Medicare Conditions of Participation.
  • Serve as a trusted resource for families, providing guidance, education, and emotional support.
