About The Position
A great healthcare facility in Sacramento, California, is seeking a Director of Patient Care Services to lead its home health team in a setting with easy access to nearby cities and outdoor attractions. This is a Full-Time, Permanent, Direct-Hire position with a schedule of Monday - Friday, 8 AM - 5 PM. The role is in a Home Health setting.
Requirements
- Must have an active and unencumbered RN License
- Must have at least 4 years of recent home health experience
- Must have previous management-level experience in home health, ideally at the manager, supervisor, or director level.
Responsibilities
- Maintain adherence to state regulations, federal laws, and Medicare Conditions of Participation.
- Direct patient care plans, admissions, re-certifications, and multi-disciplinary case conferences.
- Lead a team of clinical staff and administrative support personnel. Provide leadership, guidance, and mentorship to ensure high-quality patient outcomes and strong team collaboration.
- Oversee documentation accuracy, survey readiness, and interdisciplinary team communication.
