About The Position
American Home Health delivers compassionate, person-centered care. We're looking for a Director of Patient Care Services / Director of Nursing to lead our home health clinical team, ensure regulatory compliance, and drive quality outcomes that help patients heal safely in the place they love most.
Requirements
- Active RN license in California (BSN required; MSN or equivalent preferred)
- 5+ years of progressive nursing leadership experience in home health, including oversight of multi-disciplinary teams
- Deep knowledge of Medicare home health regulations, OASIS, and PDGM reimbursement
- Proven track record in survey readiness, quality assurance, and performance improvement
- Exceptional communication and relationship-building skills with staff, physicians, and referral partners
Nice To Haves
- Certification in Home Health Nursing (e.g., COS-C, CHPN) or Nursing Administration
- Experience with EMR systems used in home health (e.g., Homecare Homebase, PointClickCare)
Responsibilities
- Lead and manage all clinical operations
- Supervise, mentor, and develop clinical managers and field staff, fostering a culture of collaboration and excellence
- Ensure full compliance with Medicare, Medi-Cal, and state home health regulations, including Conditions of Participation
- Oversee OASIS accuracy, care planning, and documentation to support quality outcomes and appropriate reimbursement
- Partner with executive leadership on strategic growth, performance metrics, and process improvements
- Serve as a clinical resource and trusted point of escalation for patients, families, and referral sources
