About The Position

Connected Home Health is currently seeking an entrepreneurial minded Director of Nursing to join our team of dedicated professionals who want to make a difference in the lives of our patients, their families, and our community. The Director of Nursing is responsible for the leadership, management, and oversight of the clinical nursing team and day-to-day nursing operations within home health services. Reporting to the Director of Clinical Services, the Director of Nursing ensures the delivery of high-quality, patient-centered care, supports clinical staff performance, and maintains compliance with all applicable standards and regulations. The Director of Nursing implements and evaluates nursing-specific goals and objectives that align with the broader clinical strategy established by the Director of Clinical Services and supports the overall mission and philosophy of the organization.

Requirements

  • Must be a Registered Nurse (RN) with current licensure in the State (preferred).
  • Minimum of two (2) years of management experience in a home health or related healthcare organization.
  • Demonstrated ability to supervise and direct clinical nursing personnel.
  • Strong leadership, communication, and organizational skills.
  • Knowledge of state and federal regulations related to home health and nursing practice.
  • Ability to collaborate effectively with interdisciplinary teams and leadership.
  • Must be a licensed driver with an insured and reliable automobile in good working order.

Responsibilities

  • Provides direct oversight of all nursing personnel and nursing-related clinical services.
  • Coordinates and oversees all direct and indirect patient services provided by nursing staff.
  • Ensures a Clinical Manager/Supervisor is available during all operating hours.
  • Provides guidance and counseling to Clinical Supervisors and nursing staff to improve clinical care delivery and performance.
  • Assists Clinical Supervisors in managing clinical nursing teams and care planning activities.
  • Oversees the development, implementation, and updates of individualized plans of care, ensuring appropriate nursing involvement and execution.
  • Supports communication with physicians and interdisciplinary team members regarding nursing care and patient needs.
  • Provides consultation and support in assessment, planning, implementation, and evaluation of patient and family/caregiver care.
  • Ensures safe, effective, and evidence-based nursing care practices.
  • Hires, supervises, evaluates, and terminates nursing personnel in collaboration with the Director of Clinical Services.
  • Conducts clinician performance evaluations annually, or more frequently if indicated.
  • Assists Clinical Supervisors in developing skills in evaluating clinician performance.
  • Develops, implements, and evaluates orientation programs for new nursing staff.
  • Plans and implements in-service and continuing education programs for nursing personnel.
  • Ensures nursing services comply with all local, state, and federal laws and regulatory requirements.
  • Oversees the maintenance of patient clinical records in compliance with applicable standards.
  • Participates in performance improvement initiatives, including productivity reviews and quality outcomes monitoring.
  • Assures quality and safety in the delivery of nursing services.
  • Monitors operational indicators (e.g., census, patient acuity) and communicates implications for nursing staffing and resource needs to the Director of Clinical Services.
  • Oversees maintenance of adequate nursing-related supplies and equipment.
  • Supports budget preparation and management related to nursing services in collaboration with the Director of Clinical Services and Executive Director.
  • Assists the Director of Clinical Services in implementing clinical goals, policies, and procedures.
  • Contributes to the development and evaluation of organizational and clinical programs.
  • Stays current with changes in nursing and home health care and shares relevant updates with nursing staff.
  • Promotes home health services and supports referral development within the community.
  • Performs other duties as assigned by the Director of Clinical Services or Executive Director.
  • In designated circumstances, may assist in leadership coverage as directed by executive leadership.

Benefits

  • Salary: commensurate with experience and will fall between $110,000-$120,000 per year.
