Requirements

Active Registered Nurse license in the state of practice
Strong leadership experience in home health, hospice, or a similar healthcare setting
Excellent understanding of state and federal home health regulations
Experience managing clinical teams and improving quality of care
Strong communication, organization, and problem-solving skills
Ability to mentor and support a diverse team
A passion for patient safety, excellence, and continuous improvement
A positive attitude and a desire to grow with an expanding agency

Responsibilities

Lead and support the clinical team to deliver exceptional, patient-centered care while overseeing all nursing operations and ensuring compliance with home health regulations and standards.
Manage the creation and oversight of 485 plans of care, review clinical documentation for accuracy, completeness, and regulatory compliance, and monitor patient care plans and outcomes to ensure quality and effectiveness.
Recruit, hire, onboard, train, and mentor Field Registered Nurses (RNs), providing ongoing coaching and performance support to foster clinical excellence.
Develop, implement, and maintain clinical policies, procedures, and quality improvement initiatives to promote best practices and continuous agency improvement.
Collaborate with physicians, administrators, and the interdisciplinary team to coordinate patient care, resolve escalated patient or family concerns, and support positive clinical outcomes.
Prepare for surveys, audits, and regulatory reviews while helping shape the agency's clinical direction and supporting its continued growth and expansion.
