About The Position

Spring Oak Senior Living is seeking a compassionate and dependable Director of Nursing / Director of Health & Wellness (RN) to lead our Assisted Living and Memory Care Wellness team at Spring Oak at Berlin. This position is responsible for the overall leadership, quality, compliance, and delivery of resident care.

Requirements

  • Current, unrestricted Registered Nurse (RN) license in the State of New Jersey is required.
  • At least two years of nursing leadership or supervisory experience.
  • Strong knowledge of resident assessments, care planning, medication management, infection control, clinical documentation, and New Jersey assisted living regulations.
  • Proven ability to lead, develop, and hold clinical team members accountable.
  • Strong communication, organization, clinical judgment, and problem-solving skills.
  • Experience with electronic medication administration records, including eMAR.

Nice To Haves

  • Assisted living, memory care, senior living, or long-term care experience is strongly preferred.

Responsibilities

  • Lead all Wellness department operations, including staffing, scheduling, supervision, training, coaching, performance management, accountability, and corrective action.
  • Conduct initial resident assessments, ongoing reassessments, and evaluations following changes in condition, hospitalizations, or rehabilitation stays.
  • Determine clinical appropriateness for admission and return to the community.
  • Develop, implement, and update individualized resident care plans.
  • Oversee medication management, physician orders, eMAR, clinical documentation, infection control, and staff competency.
  • Monitor resident changes in condition and ensure timely communication with families, physicians, and appropriate healthcare providers.
  • Coordinate care with physicians, pharmacies, hospitals, rehabilitation facilities, hospice, home health, therapy providers, and other clinical partners.
  • Investigate and manage falls, medication errors, injuries, behavioral concerns, and other resident incidents, including documentation, notification, follow-up, and preventive action.
  • Oversee Memory Care services, including dementia-related needs, behaviors, safety, supervision, and appropriate care interventions.
  • Ensure compliance with New Jersey regulations, company policies, and clinical standards while maintaining ongoing survey readiness.
  • Lead quality assurance and clinical improvement efforts to support positive resident outcomes.
  • Participate in after-hours and on-call clinical responsibilities and ensure appropriate Wellness staffing coverage.
