About The Position

The Director of MDS conducts and coordinates the development, completion, and transmission of resident assessments in accordance with state requirements and the facility’s policies and goals. The role coordinates preliminary and comprehensive assessments, including Care Area Assessments (CAAs), triggers, and plans of care, and helps ensure that each resident’s care plan addresses their medical, nursing, mental, and psychosocial needs. An incumbent who is an LVN performs these duties under RN supervision.

Requirements

• Previous MDS experience required.
• Nursing degree from an accredited college or graduation from an approved LVN or RN program.
• Current, unencumbered license to practice as an LVN in California.
• Current valid CPR certification.

Nice To Haves

• Two to four years’ experience providing direct care to multiple patients or residents preferred.
• Previous experience in a skilled nursing environment desirable.
• Supervisory experience preferred.
• RN license preferred.

Responsibilities

• Conduct and coordinate preliminary and comprehensive resident assessments (MDS) in accordance with applicable rules, regulations, and guidelines, including implementation of CAAs, triggers, and plans of care.
• Ensure care plans include measurable objectives and timetables addressing residents’ medical, nursing, mental, and psychosocial needs; involve appropriate health professionals in assessments; and develop the required activity schedule.
• Ensure the initial resident assessment is completed within 14 days of admission and each portion is signed and dated by the person who completed it.
• Conduct or coordinate resident interviews for assessments.
• Help develop and update comprehensive resident assessments and care plans for each resident.
• Ensure assessments and care plans are updated quarterly, annually, and upon significant changes in condition.
• Evaluate each resident’s condition and pertinent medical data to determine whether the admission assessment needs amendment.
• Perform other tasks as assigned.
