About The Position

The Director of Intervention & Stabilization leads two of DSI's most critical and complex service functions. This role connects members and community residents to the resources, case management, and rural outreach services they need to maintain independence. It also oversees the Stabilization tier, which delivers services to members requiring the most intensive support. This is a role that requires both strong human services leadership and organizational discipline to build something new while managing something active. The main office for this position is located in St. George, SC. Traveling to the Summerville Center will be required.

Requirements

Demonstrated knowledge of the human services or healthcare landscape in Dorchester County or a comparable South Carolina community preferred.
Bachelor's degree required; Master's degree in social work, human services, public health, nonprofit management, or a related field strongly preferred.
Minimum five years of progressively responsible experience in case management, human services, or community outreach, with at least two years in a supervisory or leadership role.
Experience with CLTC, Medicaid waiver programs, or home and community-based services preferred.
Strong organizational and project management skills; ability to manage an active Intervention program while simultaneously building a new service line.
Ability to manage a departmental budget.
Excellent communication and relationship-building skills.
Proficiency in Microsoft Office and Google Workspace; experience with case management or care coordination software preferred.
Ability to handle sensitive member information in compliance with confidentiality standards and HIPAA.
Valid South Carolina driver's license with no more than six points in violations — regular travel throughout Dorchester County required.
The employee is routinely required to sit, stand, reach, and lift items weighing up to 30 pounds.
Must be able to communicate clearly in English, verbally and in writing.

Responsibilities

Lead DSI's Intervention service tier — overseeing case management, rural outreach, social work services, and resource navigation across both the Summerville and St. George service areas.
Directly supervise the Case Manager, and Rural Services Manager, providing regular coaching, performance feedback, and professional development support.
Ensure the Case Manager is maintaining accurate, timely case records and managing an appropriate caseload with clear protocols for intake referrals and warm handoffs to the Stabilization tier.
Own DSI's rural outreach strategy as an organizational priority — directing the Rural Services Manager's work and ensuring that seniors in the western and upper parts of Dorchester County are actively identified and connected to services.
Oversee the Case Management function, ensuring members with complex needs receive appropriate professional support and that mandatory reporting obligations are fulfilled.
Help build and maintain DSI's community partner network — NGOs, health systems, government agencies, faith communities — ensuring referral relationships are current and productive.
Establish and maintain clear protocols for receiving referrals from the Prevention tier as member needs escalate.
Ensure that care plans, scope-of-practice boundaries, and all protocols are documented and followed consistently.
Escalate any compliance concern, member safety incident, or scope-of-practice question to the CEO/Executive Director immediately — never make independent clinical judgments on behalf of the program.
Represent DSI in community coalitions, interagency meetings, and collaborative planning processes relevant to Intervention and Stabilization services as requested by the CEO/Executive Director.
Assist with building relationships with healthcare partners — hospitals, clinics, MUSC, home health agencies — that will be important referral sources and collaborative partners for the homecare program.
Ensure that DSI's Intervention and Stabilization services are known and accessible to the populations that need them most, including those in rural and underserved parts of Dorchester County.
Prepare regular reports on Intervention tier activity, caseload, referral volume, partner engagement, and rural outreach metrics for the CEO/Executive Director.
Ensure that all data collection, reporting requirements, and performance metrics are met accurately and on time.
Contribute to grant applications, funder reports, and board presentations as requested by the CEO/Executive Director.
Participate in director-level leadership team meetings and contribute to DSI's strategic planning process.
Maintain a regular field presence — at both centers, in rural communities, and with community partners — not primarily office-based.
Represent DSI professionally in all interactions with members, community partners, government agencies, regulators, and the public.
Assist with other duties as assigned.
Attend board meetings as directed by the CEO/Executive Director.
Adhere to all DSI policies, procedures, and ethical standards — including member confidentiality, mandatory reporting obligations, and compliance.
Model the organizational values and professional standards expected of all DSI staff.
