About The Position

Crossroads Health, a 501(c)(3) non-profit organization with facilities located in Mentor, Painesville, and Cleveland, serves Northeast Ohio communities with comprehensive behavioral and primary integrated healthcare, early childhood services, extended housing and recovery services—no matter one’s ability to pay. Our mission is to provide hope, healing, and healthcare to everyone. We are looking for a mission-driven, detail oriented and personable individual to fill the position of Director of Integrated Healthcare. This position will work closely with members of the executive management team, clinical directors and supervisors, quality improvement, administrative directors and other key constituents and partners. This position oversees the compliance and operational functioning of the Health Center which includes primary care, psychiatric services, pharmacy, nursing and clinical support services. The Director of Integrated Health will oversee the operations of pharmacy services and all other specialty programs that relate to the Health Center, including the clinical and compliance of operations related to the day-to-day functions of the Health Center. The Director of Integrated Healthcare promotes healthcare transformation and innovative models for integrated primary and behavioral health care, including performance on Quality Measures and Health Center KPI’s. Responsibilities include oversight of health center operations, health center strategy, school base health center operations and other key strategic initiatives related to the health center. The Director of Integrated Healthcare will act as the organization Infection Prevention staff.

Requirements

  • Bachelor’s degree in clinical field required with a current license to practice, RN (preferred).
  • Three (3) years of experience in a clinic or managed care environment, health care payor systems, or integrated care initiatives required
  • Five (5) years of experience in a healthcare system, nonprofit management or a related field.
  • Understanding of Health Center compliance, primary care models (PCMH), patient-centered care, value-based care.

Nice To Haves

  • Master’s degree in business administration, Public Health or Health Administration, (preferred).
  • Five (5) years of full-time paid experience in program development/program consultation preferred.
  • LEAN Six Sigma Yellow Belt (preferred).

Responsibilities

  • Build strong relationships with internal leaders, and external partners to drive the strategic initiatives of the health center. Act as the liaison across departments to ensure seamless, patient-first operations for the Health Center.
  • Foster a culture of innovation and accountability focused on patient’s outcomes and operational excellence. Provides leadership oversight for the organization’s Uniform Data Set reporting, including working with quality on workflow development and training of health center staff to meet quality measures.
  • Partner with internal leadership to develop, initiate and monitor Health Center KPI’s and provide data to leadership and clinical staff.
  • Ensure the day-to-day success of the Health Center Operations, including managing relationships with clinical vendors. Develop and execute strategies to enhance care delivery, population health outcomes, and patient engagement within the Health Center.
  • Provides healthcare integration leadership for operations; developing, communicating, executing, and sustaining strategic initiatives; creative and economic endeavors; space utilization; communications; and planning.
  • Provides fiscal planning, monitoring, and oversight of services provided by the Health Center to meet financial goals. Oversee the annual budget process in coordination with the VP of Operations, submit requests, monitor expenses and resource allocation.
  • Act as the organization Infection Prevention staff.
