PRC Baker Places is a non-profit organization dedicated to serving the San Francisco community through legal advocacy, workforce development, financial services, and residential treatment programs. We support formerly homeless adults living with co-occurring mental health and substance use disorders by providing a continuum of integrated direct care services. All of our programs are grounded in a social rehabilitation philosophy that integrates harm reduction and daily living skills within a safe and supportive environment. Job Title: Director of Intake & Placement Division/Department Programs Position Overview The Director of Intake & Placement coordinates the flow of clients into all Baker Places programs from external and internal referral sources. The Director interviews clients in different settings including but not limited to jails and inpatient hospital units. The Director ensures clients are placed in appropriate levels of care. The Director participates in LPHA on-call duties and assists in fostering internship/externship/practicum experience. The Director of Intake & Placement partners closely with the Residential, Respite, Leland House, and Case Management Program Directors to achieve an excellent standard of client care and staff support.

Requirements

  • Actively licensed in California as an LCSW, LMFT, Psy.D. or other Licensed Practitioner of the Healing Arts for a minimum of 2 years upon hire required.
  • Minimum 2 years’ experience in an adult/older adult mental health setting required.
  • Must be able to obtain and maintain CCL and jail clearance.
  • Strong written and verbal communication skills.
  • Understanding of and commitment to Social Rehabilitation.
  • Ability to work in different locations and settings required.
  • Demonstrated effective time management with the ability to track and meet deadlines while also maintaining accuracy.
  • Ability to develop, maintain, and adhere to processes.
  • Proficiency in Microsoft Office products required.
  • Ability to establish collaborative partnerships with supervisees, colleagues, and community organizations.
  • Knowledge and understanding of trauma-informed and other evidence based therapeutic principles/practices.
  • Flexibility, adaptability, and ability to focus attention on workflow processes, procedures, budgetary guidelines, and documentation/productivity standards.
  • Must be available to work flexible hours occasionally – including possible nights and weekends.
  • Exhibit exemplary customer service, compassion, and care in the execution of all duties and interactions.

Responsibilities

  • Conducts and documents interviews with clients who may be currently hospitalized, incarcerated, homeless or participating in other programs.
  • Collects, assembles, reviews, clinically analyzes, completes documents as needed, and delivers all clinical information and documentation necessary for the destination program.
  • Completes documentation to the highest standards – timeliness and quality.
  • Supervises the Intake Specialist.
  • Assess the diagnosis and clinical appropriateness of clients entering Baker Places programs.
  • Maintain data in the agency database including key markers.
  • Adheres to HIPAA and other confidentiality requirements.
  • Identifies and conducts outreach to internal departments, potential community-based agencies, government agencies, and jail diversion programs.
  • Attend, participate in, and organize internal meetings as needed.
  • Attend, participate in, and organize flow and other meetings with our partners.
  • Participates in LPHA on-call duties.
  • Supervises interns/externs and those completing practicum programs.
  • Conduct intakes onsite at programs as needed.
  • Responsible for maintaining occupancy levels across all programs.
  • Manage and develop relationships with community partners and funders.
  • Coordinate flow of clients within organization’s internal continuum of care.
  • Participate in Agency Admin on-call rotation.
  • Additional duties as assigned.
