About The Position

The Director of Housing and Care Management provides strategic oversight of housing-related case management services for clients with behavioral health challenges, including those with co-occurring disorders. This role involves direct leadership of the Care Management team and collaboration with the MDC Property Management Team to ensure the delivery of comprehensive support and services. The Director will run reports to ensure case managers maintain efficient and effective methods of data entry. This role also involves maintaining and enhancing rigorous processes and procedures to support staff in assisting clients to achieve and sustain independent living, and managing teams providing coordinated support services and housing-related counseling. The Director will oversee the integration of best practices such as motivational interviewing, trauma-informed care, goal planning, and progress measurement, and ensure the implementation of person-centered assessments and strength-based housing care plans, coordinating with property managers, family members, and other key stakeholders. Additionally, the Director will monitor housing services for individuals with behavioral health needs, including mental health and substance use disorders, ensuring timely, accurate documentation and compliance with billing requirements. The role requires an understanding of landlord tenant laws, property management and leasing of various types of units, Project Base, Property Base, and affordable units, as well as Housing First principles, Housing contract management and the ability to work with property managers to report data to leadership and funders in a timely manner. The Director will lead and develop the Care Management team, ensuring high service quality and coordination with MDC and community partners, and oversee recruitment, training, evaluation, and recognition of staff, maintaining adherence to HR policies and contractual agreements. Finally, the Director will utilize data to drive program outcomes, focusing on positive change, productivity, and quality, and ensure all aspects of documentation, data entry, and reporting are executed timely and accurately. The Director will also develop and monitor the department’s budget, managing resources effectively to support program objectives.

Requirements

  • Bachelor's degree required
  • 5 years of supervisory/management experience
  • Experience in high-level program management; maintaining and leading others toward fidelity to service delivery models and best practices.
  • Experience advocating on behalf of individuals with complex needs, such as medical, mental health, and substance use disorder.
  • Experience working with the Housing First principle, subsidized housing, and permanent supportive housing, transitional housing, and affordable housing.
  • Experience with housing contract management and yearly reporting requirements to various funders.
  • Superb organizational skills with ability to prioritize effectively and meet deadlines.
  • Proven experience with treatment, recovery, and homeless issues/services/etc.
  • Proven experience with Motivational Interviewing, Trauma Informed Care, Patient/Client-Centered Approaches, etc.
  • Demonstrated experience with employing rigor as it relates to service delivery, charting, billing, regulatory practices, and contract adherence.
  • Familiar with incorporating regulations issued by the government and funders, the WAC (Washington Administrative Code), etc into practices, policies, procedures, and oversight.
  • Familiar with navigating database systems to ensure data integrity and completion.
  • Ability to effectively collaborate with community partners.
  • Knowledge of Medicaid
