Director of Clinical Services
About The Position
Rocky Mountain Care is looking for an experienced Home Health Clinical Director / Director of Clinical Services to lead our Home Health and Hospice team in Utah County. You'll have an established mentorship structure, direct access to the Administrator, and a strong, supportive team around you—while having the opportunity to help us build toward a level of clinical excellence that hasn't reached in our market yet. Requirements
- Current RN license in good standing
- Strong hands-on experience with OASIS
- Understanding of how clinical practice, documentation, patient outcomes, CMS quality measures, and regulatory compliance work together
- Current Utah RN license
- Minimum of two years management experience
- Critical thinking/problem-solving skills
- Excellent interpersonal skills, proven ability to create and maintain a cohesive, effective team
- Available at all times during operating hours
- Understand business growth, financial management, personnel management
- Must be organized, able to multitask, and delegate when appropriate
Nice To Haves
- Hospice experience is preferred, but not required.
Responsibilities
- Oversee Home Health and Hospice clinical operations
- Partner directly with the Administrator to drive clinical quality and patient outcomes
- Ensure OASIS accuracy and clinical documentation
- Manage CMS quality measures and 5-Star performance
- Maintain Medicare and regulatory compliance
- Develop and hold clinical staff accountable
- Lead quality improvement initiatives
- Oversee case management and plan-of-care
- Drive staff engagement and retention
- Foster interdisciplinary collaboration
- Implement strong, consistent clinical processes
