About The Position

Carefirst Seniors & Community Services Association is a non-profit charitable community services agency established since 1976. Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support and home care services, covering the whole Greater Toronto Area. With a staff of 550 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges. 3,000 of whom are "home-bound" and frail. We are currently seeking a Director of Client Care to fill a vacant position at our Management Team.

Requirements

  • Master degree in a related social/health sciences discipline
  • A minimum of five years of progressive leadership responsibility and project management, preferably in community health and/or social services
  • Excellent leadership, organizational, presentation, interpersonal and problem-solving skills
  • Sound business and technical acumen with ability to exercise innovation and initiative in identifying business opportunities
  • Experience with stakeholder management and proven ability to establish solid relationship with sponsors and key stakeholders
  • Proven ability to mobilize and motivate teams; set direction and approach; resolve conflict; deliver tough messages with grace; execute with limited information and ambiguity
  • Manage in a fast-paced, dynamic and ambiguous work environment
  • Ability to work independently and effectively with limited supervision
  • Demonstrated skills in financial, human resource and relationship management
  • Extensive knowledge of project management processes, systems development methods, organizational change management concepts, and business process improvement methods
  • Excellent project management skills, utilizing project planning techniques and automated project planning tools and templates, e.g. scope statements, requirement analysis, lessons learned, Gantt charts, Project Network Diagrams, Critical Path Analysis, etc.
  • Ability to communicate at all levels with clarity and precision, both written and verbally
  • High proficiency in English and a second language commonly used by our clients is an asset
  • Current vulnerable sector check (within one year from the date of hiring)

Responsibilities

  • Provide leadership and strategic directions to the managers, program staff, and health care professionals, in supporting multi-program delivery and ensuring the provision of client care services and development are in accordance with Carefirst’s policies and procedures and related legislations
  • Develop and execute strategies to expand the organization's reach and impact, including new services implementation, participation in OHTs, collaborations, and outreach initiatives
  • Plan, evaluate and monitor the multiple Service Departments’ financial and operational performance and ensure cost-effective processes and procedures are in place
  • Monitor and report on key performance indicators to track progress towards organizational strategic goals and decision-making
  • Ensure multiple Service departments including Transitional Care Centre, Overnight Stay Program, Adult Day Program and Community Support Services adhere to organizational safety and quality standards and ensure staff and clients’ compliance with Carefirst policies, guidelines and procedures related to patient/client safety
  • Manage and coordinate cross-functional teams to achieve project goals and objectives
  • Develop and maintain good relations with the major community stakeholders and health care partners
  • Represent the organization at public events, conferences, and other relevant forums to promote the mission and work of the organization
